The space between them is filled with fluid, allowing them to glide smoothly along each other when you breathe, similar to oil in a machine. When one or both layers swell, they rub painfully against each other. Pleurisy is characterized by a sharp, stabbing, or knife-like chest pain that radiates from a single location. It gets worse when you cough or breathe deeply, and it can spread to your shoulder or back. To avoid the pain, you'll probably find yourself breathing carefully.
